Reusable Microsoft Excel problem-solving workbook

RCCA & 8D Toolkit
from issue to verified closure

A structured root cause corrective action workbook for engineering and manufacturing teams. Every issue starts with risk screening and a three-legged 5 Why covering direct cause, detection cause and systemic cause, with defined triggers to escalate the investigation into a full D1–D8 process when required.

Use the lightest method that is justified by the risk.

The toolkit is designed so a lower-risk issue does not automatically become a heavyweight 8D. Triage screens severity, likelihood and mandatory escalation conditions. Every issue begins with the initial three-legged root cause analysis; higher-risk, safety, contractual, repeat or otherwise triggered cases move into the full 8D route.

Three cause legs

  • Direct cause — what physically or operationally created the defect?
  • Detection cause — why did the intended control miss it, or why was no control present?
  • Systemic cause — what management-system weakness allowed the creation or escape controls to be inadequate?
  • Each chain includes evidence, cause validation and review gates.

Escalation into full 8D

The Triage sheet includes configurable local risk thresholds plus mandatory escalation questions. Full 8D can be triggered by safety/legal/regulatory concern, customer or contract requirement, repeat failure, failed containment, external escape, known complexity or an investigation-lead decision.

The workbook explicitly states that local threshold settings should be approved against the organisation’s own QMS and customer requirements.

From problem definition to effectiveness evidence.

Investigation workflow

  • People — shared roles and stable person IDs
  • Problem — enter the case information once
  • Triage — risk screening and route selection
  • 3 Leg RCA — evidence-based cause analysis
  • Actions — permanent corrective actions and acceptance criteria
  • Verification — sustained effectiveness and independent review
  • 8D — D1 to D8 when escalation is required
  • Closeout and Report — controlled closure and output

Designed to avoid weak RCA

The workbook does not force five answers simply because the method is called 5 Why. It asks for a supported stopping point, cause proof and a validation test or evidence reference. It also separates direct, detection and systemic mechanisms rather than allowing “human error” to become the final answer.

A fictional worked example and a Guide worksheet are included to explain the intended workflow.

Before you purchase.

Does every issue require a full 8D?

No. The toolkit uses triage and an initial three-legged 5 Why for every issue. Full 8D is used when risk, mandatory triggers, the quality of the initial analysis or later effectiveness evidence requires escalation.

What worksheets are included?

Report, Triage, Problem, 3 Leg RCA, Actions, Verification, 8D, Closeout, Worked example, Guide and People.

Does it use macros?

No. The workbook is macro-free and requires no external data connections or subscription.

What do I receive after payment?

The original editable Microsoft Excel XLSX workbook through the verified order page. It is a one-off digital purchase.

Licence and delivery.

The standard purchase gives one legal entity a non-exclusive licence to use and adapt the workbook for its own internal business activities. Completed outputs may be shared where reasonably necessary, but the editable source workbook cannot be resold or distributed as a standalone resource.

After successful Stripe payment, the order page verifies the payment and makes the original Excel workbook available for download.

RCCA, 5 Why or full 8D: use the method that matches the problem.

Not every issue needs a full 8D, but significant, recurring or high-risk failures usually need more than a single causal chain. The toolkit starts with direct, detection and systemic causes and escalates when the risk and evidence justify it.
